We poll Reddit on a fixed 5-minute cycle. When a new post or reply contains your keyword, the alert goes out on the following check, so it lands within a few minutes of the thing being posted.

Yes. You can point a keyword at the communities you care about, or keep it wide and rule out the ones that only ever bring noise. Most people start wide, see where their phrase actually lands, and tighten from there.
